I have a question for

I want to

Chronic care in Almere falls under the so-called “integrated care”. This means that there is intense collaboration (chain) with multiple healthcare providers to ensure care. The care is primarily provided by the Practice Support Worker for Somatics (POH-S) and the general practitioner is ultimately responsible. If you are under the chronic care of Medi-Mere, you will deal with the medical assistant, the POH-S, the general practitioner but sometimes also with the dietician, medical pedicurist, optometrist, physiotherapist, pharmacist, or perhaps a specialist in the hospital. It is important that there is clarity about your condition and how the care surrounding your condition is ensured. Hence, integrated care.

The chronic care/integrated care is applied for the following conditions:

  • Diabetes Mellitus
  • COPD/emphysema
  • CVRM (cardiovascular diseases)
  • Elderly care
